The Susquehanna Wellness Clinic, a primary care office located in Frenchville, PA will offer services starting in the fall of 2020. The facility has roots in Frenchville community as the building formerly served the community as the Susquehanna Rural Free Clinic.

    At SWC, patients are the center of our focus. The clinic serves adults patients 18 and older with an emphasis on providing quality and meaningful care. We take a comprehensive approach to care by providing chronic care management education, telehealth and telemedicine services as well as onsite health and wellness programs. Other services we offer include patient transportation for elders to medical appointments, in home services to patients physically unable to come into the clinic and remote visits to senior centers and high rises. Additionally, SWC participates in most managed care plans, processes most insurances and is able to accept credit card and cash payments.
